COVER SHEET EXCEPTIONS

Cover sheets are not required for the following:
- confidential special proceedings and confidential juvenile proceedings;
- small claims;
- service documents, Notices of Hearing, Calendar Requests and routine discovery documents;
- claims of lien and other lienholder miscellaneous judgments (for example, federal tax liens, hospital liens and materialmans liens);
- voluntary support agreements and simple orders of paternity;
- pro se
domestic violence filings using form AOC-CV-303;
- post-disposition child support enforcement filings;
- "open and shut" estates matters and estates matters the nature of which are clear from their face (for example, a surviving spouse’s year’s allowance or a ninety-day inventory);
- special proceedings matters the nature of which are clear from their face (for example, an accounting in a foreclosure or a notice of upset bid);
- a Notice of Case Selection for Arbitration, Notice of Arbitration Hearing, Arbitration Stipulation and Order, Arbitration Award and Judgment, Arbitration Request for Trial de Novo, or Application and Order for Payment to Arbitrator filed in conjunction with the court-ordered arbitration program;
- an Order on Child Custody Mediation, Order Approving Parenting Agreement, or Order to Calendar Custody or Visitation Dispute filed in conjunction with the custody and visitation mediation program;
- an Order for Mediated Settlement Conference and Trial Calendar Notice, Designation of Mediator, Report of Mediator, or Petition and Order for Relief from Obligation to Pay Mediator’s Fee filed in conjunction with the mediated settlement conference program;
- a Request for Prelitigation Mediation of Farm Nuisance Dispute, Appointment of Mediator in Prelitigation Farm Nuisance Dispute, Waiver of Prelitigation Mediation in Farm Nuisance Dispute, or Mediator’s Certification in Prelitigation Farm Nuisance Dispute filed in conjunction with the prelitigation farm nuisance mediation program; and
- motions, orders and disposition documents prepared and filed by a judge, Clerk of Superior Court, or other court official, including administrative orders (note: if an attorney or litigant prepares the filing, he or she should complete the appropriate cover sheet and present both the filing and the completed cover sheet to the judge or court official).
